29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about July 20, 2017, employees were performing servicing tasks including cleaning the asphalt off of the rollers on the Mell Roll Line. Employees were not required to control hazardous energy sources in the following sequence: 1. Prepare for shut down by identifying all of the energy sources and the method or means to control the energy. 2. Turn off and shut down the machine. 3. Physically located and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s). 4. Affix lockout or tagout devices to each energy isolating device. 5. Relieve, disconnect, restrain or otherwise render safe all potentially hazardous stored or residual energy. 4. Verify that isolation and de-energization of the machines or equipment have been accomplished. Abatement documentation is required of this item in accordance with the requirements of 1903.19(d).
