On the State Route 7 bridge, employees were working on a truck platform with a horizontal lifeline which was not designed or installed by a qualified person thereby exposing employees to a struck-by hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall shall: be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contactany lower level; a. On the State Route 7 bridge where employees worked from a platform on top of a Ford truck, the horizontal lifeline and fall protection system allowed the employees to hit the ground before being protected from the fall arrest system thereby exposing the employees to a contact hazard.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break inelevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. On the State Route 7, bridge in Shadyside, Ohio, the employees were accessing the work platform from the ground and were not provided a safe access from the ground to the fixed ladder that was four feet, four inches from the ground thereby exposing employees to a fall hazard.
