29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) B & W Welding, Inc., located on 130 CO RD 130 in Fremont, OH: On or about July 7, 2011, employees were exposed to amputation hazards associated with but not limited to the Amada RG 125 Press Brake. The employer did not assure that the press brake was adequately guarded to protect employees from its point of operation.
