29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: (a) Coater # 3, #1 upper deck, priming station #1, primer 1 exit; Guard rails were not present to prevent falls into a 8' by 32" wide hole. The lower landing surface was approximately 9'7" below. (b) Primer # 2, upper deck; Guard rails were not present to prevent falls into an approximate 8' by 91" hole. The lower landing surface was approximately 9'7" below. (c) Extruder #7, upper deck; Guard rails were not present to prevent falls into an approximate 91" by 10" hole. The lower landing surface was approximately 9' 5" below. (d) Coater # 8, coating station # 2, both sides; A guard rail was missing between the existing guard rail and the support structure, approximately 37.5" in height and 30" wide tapering to the floor at an approximate 45 degree angle. Employees would fall into the machine through the opening.
