29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Building 11, rewind area of coater 4; Lockout devices were not affixed to isolate pneumatic energy for a sled (splicing blade) during servicing operations, including but not limited to; rethreading the web and changing the sleds direction of cut. The Loparex, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147 (d)(4)(i), which was contained in OSHA inspection number 1056564, citation number 1, item number 2 and was affirmed as a final order on July 20, 2015, with respect to a workplace located at 1740 Ridgeway Street, Hammond, WI.
