29 CFR 1910.27(b)(1)(iv): Rungs, cleats, and/or steps of fixed ladders were not free of splinters, sharp edges, burrs, or projections that posed a hazard:    (b)	Nabors Drilling USA at Daybreak Federal #19 Pad, Rig #M15 in Parachute, CO: On and before 7/8/11, the employer did not ensure that all rungs, cleats, and/or steps of fixed ladders were free of splinters, sharp edges, burrs, or projections. The left side of the fixed ladders attached to the white BAR tank is bent in from impact. This condition exposed employees to a fall hazard.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts:    (a)	Nabors Drilling USA at Daybreak Federal #19 Pad, Rig #M15 in Parachute, CO: On and before 7/8/11, the employer did not protect employees from hazards created by unguarded rotating parts of the harmonic balancer. The harmonic balancer was measured to be approximately 4" x 7" opening. This condition exposed employees to an amputation hazard.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:      (a)	Nabors Drilling USA at Daybreak Federal #19 Pad, Rig #M15 in Parachute, CO: On and before 7/8/11, the employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by permit spaces. The mud tank and water tank on site had been evaluated as permit required spaces. Nabors did not identify these spaces by posting danger signs or by any other equally effective means. This condition exposed the employees to a confined space hazard.
