ALLEGED VIOLATION DESCRIPTION (A.V.D.): (NAME OF VESSEL)(LOCATION OF VESSEL)(LOCATION ON BOARD VESSEL WHERE HAZARD WAS OBSERVED)(DATE VIOLATION OBSERVED)(DESCRIBE HAZARD(S) OR POTENTIAL HAZARD(S) CREATED BY FAILING TO PROPERLY DOCUMENT A HAZARD ASSESSMENT) (IDENTIFY SPECIFIC CONDITION/OPERATION) NOTE: A HAZARD ASSESSMENT CONDUCTED ACCORDING TO THE TRADE OR OCCUPATION OF AFFECTED EMPLOYEES WILL BE CONSIDERED TO COMPLY WITH PARAGRAPH (B) OF THIS SECTION, IF THE ASSESSMENT ADDRESSES ANY PPE RELATED HAZARDS TO WHICH EMPLOYEES ARE EXPOSED IN THE COURSE OF THEIR WORK ACTIVITIES. ALLEGED VIOLATION DESCRIPTION (A.V.D.): (NAME OF VESSEL)(LOCATION OF VESSEL)(LOCATION ON BOARD VESSEL WHERE HAZARD WAS OBSERVED)(DATE VIOLATION OBSERVED)(DESCRIBE HAZARD(S) OR POTENTIAL HAZARD(S) CREATED BY FAILING TO TRAIN AFFECTED EMPLOYEES ON PPE) (IDENTIFY PPE USED)(OBTAIN A COPY OF TRAINING RECORDS, IF AVAILABLE) NOTE: TRAINING IN THE USE OF PERSONAL FALL ARREST SYSTEMS AND POSITIONING DEVICE SYSTEMS TRAINING IS COVERED IN 1915.159 AND 1915.160. (a) Employees working over and near water from floating performing ship repairs were not trained on what and when personal protective equipment, such as PFD's was requied when drowning hazards exist.
