29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  (a) Kevin Hill Marine Services - main office shop area: On or about May 16th, 2017 and at times prior thereto, the employer exposed employees to an amputation or contact with hazard on the Scotchman Iron Worker punch operation station.  (model #12012 serial # 50397M293)    (b) Kevin Hill Marine Services - main office shop area: On or about May 16th, 2017 and at times prior thereto, the employer exposed employees to an amputation hazard at the point of operation on the shearing tool on the Scotchman Iron Worker. (model #12012 serial # 50397M293)

29 CFR 1915.71(j)(2): Rails shall be of 2 x 4 inch lumber, flat bar or pipe. When used with rigid supports, taut wire or fiber rope of adequate strength may be used. Rails shall be firmly secured. Where exposed to hot work or chemicals, fiber rope rails shall not be used.  (a) Kevin Hill Marine Services - M/V Erla - N: On or about May 16th, 2017 and at times prior thereto, the CSHO observed a fall protection hazard around the crab tank entrance.  There was the potential for a 12' fall which could result in broken bones and/or hospitalization.  The fiber rope and stanchions were not of adequate strength to prevent employees from a fall.

29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2).  (a) Kevin Hill Marine Services - M/V Erla-N: On or about May 16th, 2017 and at times prior thereto, the CSHO observed a fall protection hazard around the crab tank entrance.  There was the potential for a 12' fall which could result in broken bones and/or hospitalization.  The fiber rope and stanchions were not of adequate strength to prevent employees from a fall.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  (a) Kevin Hill Marine Services - main office - On or about May 16, 2017 and at times prior thereto the CSHO observed a 110 volt light switch missing a cover.  (b) Kevin Hill Marine Services - storage room - On or about May 16, 2017 and at times prior thereto the CSHO observed a 230 volt outlet missing a cover.
