29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a.) The employer did not assure that employees observed installing new roofing material on a commercial type structure on August 23, 2011 at the address of 353 Spring Sterling Hill Dr. Findlay, OH, are protected from falls of up to 17ft while using the ladder that did not extend 3ft above the working surface. The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.1053(b)(1), which was contained in OSHA Inspection Number 311838866, citation number 1, item number 1a and was affirmed as a final order on 1/27/09
