Main St. Delta, OH; On or about August 24, 2011, the employer did not assure that the performance of several industrial truck operators was evaluated. Employees routinely operated a powered industrial vehicle for moving products to and from power presses.

29 CFR 1910.217(c)(3)(i)(e): The location of the hand controls did not allow a safe distance from the point of operation so that the slide completes the downward travel or stops before the operator can reach into the point of operation with his hands: a.) GB Manufacturing Co. located at 1120 E. Main St. Delta, OH 43515; On or about August 24, 2011, employees were exposed to amputation hazards in that the two hand controls on the 300T White Bliss mechanical power press were not located at a safe distance from the point of operation. The employer did not assure that the slide completed the downward travel before the operator could reach the point of operation.
