OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: a.) GB Manufacturing Co. located at 1120 E. Main St. Delta, OH 43607; On or about May 21, 2014, racks used for storing dies were not marked with their rated capacity, had damaged structural members and were not bolted to the floor in all places. Employees were thereby exposed to crushing hazards in the event the racks failed. Abatement Note: Among others, feasible methods to abate this hazard include placing capacity labels in the appropriate locations for each rack, bolting down each rack where a hole is provided, monitoring rack damage, and ensuring storage racks are not loaded beyond their rated capacities, following the American National Standards Institute/Specification for the Design, Testing, and Utilization of Industrial Steel Storage Rack, MH16.1:2008, 1.4.2 Plaque, 1.4.7 Column Base Plates and Anchors, and 1.4.9 Rack Damage.
