29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation, 1910.217 of this part:  (a) GB Manufacturing Company, on or about December 18th, 2018 at the address of 100 Adams Street, Delta, OH did not ensure the point of operation on the spot welder (W/C 420.1) in the Adams St. building was adequately guarded.  An employee was operating the welder in this condition and cycle the machine resulting in a crushing injury.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    (a) GB Manufacturing Company, on or about December 18th, 2018 at the address of 100 Adams Street, Delta, OH: The employer did not ensure that employees were adequately trained to follow the procedures outlined by the employer's energy control program for locking out the press welders.  Employees were exposed to pinch point hazards resulting in injuries such as amputations of the fingers and thumbs while performing set ups and change overs on the press welders in an energized condition.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) GB Manufacturing Company, on or about December 18th, 2018 at the address of 100 Adams Street, Delta, OH:  The employer did not ensure that the energy control program was applied and done in the sequence required.  The employer did not ensure that the energy control program was implemented in order to protect employees from the hazards of energized equipment such as but not limited to the spot welders.  Employees performing set up activities, which involves the changing of the electrodes, without the benefit of energy isolation were were exposed to pinch point hazards at the point of operation.
