The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants, chemical state and physical form: a) Jobsite: On August 31, 2011, and times prior thereto, employees were not wearing respiratory protection while directed to cut concrete with a Multiquip Slab Saver floor saw in an environment that had not been adequately evaluated for hazards such as crystalline silica in the concrete dust. Note: Abatement certification is required for this item.
