29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) E & J Construction, located at worksite on 765 South Buchanan Street in Fremont, OH: On or about August 10, 2011, the employer did not provide three employees with fall protection while applying new roofing material to a steep sloped commercial structure. The pitch was 6/12 on the west side of the structure and 5/12 on the south side. Employees were exposed to a 10' ft fall onto the pavement below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a) E & J Construction, located at worksite on 765 South Buchanan Street in Fremont, OH: On or about August 10, 2011, the employer did not ensure that employees were trained in the proper application of personal fall protection equipment while working on steep sloped commercial structure. Employees were exposed to a fall height approximately 10 feet from the pavement below.
