29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery,equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    a) In the outside piping area employees were exposed to floor holes in the grating during the maintenance and/or inspection of equipment in this area.    b) In the mixing area employees were exposed to floor holes in the grating during the maintenance and/or inspection of equipment in this area.    c) In the shaker area employees were exposed to floor holes in the grating during the maintenance and/or inspection of equipment in this area.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall beguarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance toa ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) In the basement area an elevated platform for the Roller Mill was not equipped with a guard railing system to prevent employees to falls in excess of eight feet.    b) In the shaker area an elevated platform was inadequately guarded in that the railing did not not fully enclose the platform/working surface exposing employees to falls in excess of eight feet.

29 CFR 1910.27(d)(2):     When ladders are used to ascend to heights exceeding 20 feet (except on chimneys), landing platforms shall be provided for each 30 feet of height or fraction thereof, except that, where no cage, well, or ladder safety device is provided, landing platforms shall be provided for each 20 feet of height or fraction thereof. Each ladder section shall be offset from adjacent sections. Where installation conditions (even for a short, unbroken length) require that adjacent sections be offset, landing platforms shall be provided at each offset.        a) Ladder systems next to the Man Lift were not equipped with a landing and/or cage when employees had to climb in excess of 30 feet.    b) Ladder system on the second floor was not equipped with a landing and/or cage when employees had to climb in excess of 30 feet.    c) Ladder system on the third floor was not equipped with a landing and/or cage when employees had to climb in excess of 30 feet.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a) Flexible cord attached to the material heating element was not equipped with strain relief to prevent movement and/or damage to conductors.
