29 CFR 1910.23(a)(8): Floor hole(s) into which persons could accidentally walk, were not guarded by standard railings with standard toe boards on all exposed sides or by floor hole covers of standard strength and construction:    AVD: Navy Exchange OLF Imperial Beach warehouse upper level  On or about September 26, 2011, Employees were exposed to falls greater than four feet to the lower level  between the merchandise shelves when the floor hole covers were not in place or railings installed.    The Navy Exchange was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(8), which was contained in OSHA inspection number 315092650, citation number 1, item number 1, and was affirmed as a final order on March 30, 2010, with respect to a workplace located at Building 36, 13th Street, Imperial Beach, CA 91932.
