29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes(including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about September 19, 2011, at the above addressed jobsite, employees and subcontractor employees on the sixth (6th) floor were exposed to falls of nine (9) feet eleven (11) inches created by the presence of an unprotected hole where the tower crane penetrated the floor.
