29CFR 1910.178 (l)(4)(iii): The employer did not conduct an evaluation of each powered industrial truck operator's performance at least once every three years. a) Westport Axle Corp., Avon OH: On or about October 4, 2011, the employer did not assure that an evaluation of several industrial truck operators performance was conducted. Employees routinely operated a powered industrial vehicle for moving products from warehouse shelves and for loading and unloading semi trucks. Employees work in a small dock area and are at risk of serious struck-by injuries if the vehicle was operated unsafely.
