29 CFR 1904.32(a)(2): At the end of each calendar year, an annual summary was not created for injuries and illnesses recorded on the OSHA 300 Log: a. Windham, NH - An annual summary (OSHA 300A or equivalent) was not created for this establishment for 2010. Abatement Note: A separate OSHA Log and Summary must be kept for each establishment (physical location) that is expected to be in operation of one year or longer. In addition, each summary form must include the total of the columns from the 300 Log, the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log. Finally, the summary must be certifid by a company executive as defined in 1904.32, and posted in the establishment from February 1 to April 30 of the year following the year covered by the records.
