29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Webster Industries, Inc. - Tiffin, Ohio: On or about October 31, 2011, the employer did not assure that the de-burring tools, known as the handi-burr, were guarded to protect employees from its rotating parts. b) Webster Industries, Inc. - Tiffin, Ohio: On or about October 28, 2011, employees operating the Hass CNC machine #12-630 (s/n 15951) were exposed to injuries from the machines rotating parts and/or flying chips. The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation. c) Webster Industries, Inc. - Tiffin, Ohio: On or about October 31, 2011, employees operating the Hass CNC machine #12-035 (s/n 67632) were exposed to injuries from the machines rotating parts and/or flying chips. The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation. d) Webster Industries, Inc. - Tiffin, Ohio: d) On or about October 30, 2011, employees operating the Mazak Corp. CNC machine #12-211 (s/n 68735) were exposed to injuries from the machines rotating parts and/or flying chips. The machine was not adequately guarded in that employees were not prevented from access the hazard areas during operation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
