29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: Webster Industries, Inc. - Tiffin, Ohio: On or about July 30, 2020, the employer did not ensure that employees who setup and service the Milko-7 milling machine (#12-463) were trained in the skills required for the safe application, usage, and removal of energy controls. Employees that setup the machine's cutting wheel, part fixture, and changed cutting inserts were not trained to isolate and/or render energy sources safe.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Webster Industries, Inc. - Tiffin, Ohio: On or about July 30, 2020, employees were exposed to pinch point and rotating part hazards in that the Milko-7 milling machine (#12-463) was not locked out prior to machine setup tasks. The employer did not ensure that energy control application steps were implemented to control the machine's pneumatic and electrical/mechanical energy sources prior to the servicing tasks; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Affix lockout device(s) to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished. b. Webster Industries, Inc. - Tiffin, Ohio: On or about June 30, 2020, employees were exposed to rotating part hazards in that the Milko-7 milling machine (#12-463) was not locked out prior to changing the machine's blade inserts. The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical energy sources prior to the servicing tasks; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Affix lockout device(s) to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished.

Webster Industries, Inc. - Tiffin, Ohio: On or about July 31, 2020, employees who operated the Milko-7 milling machine (#12-463) were exposed to caught-by hazards and amputation injuries in that the rotating grinding/cutting wheel was not guarded.
