29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a. Bagging Building/Northside Conveyor's line - On or about November 3, 2011, employees were required to perform maintenance and/or service on the electrical conveyor's motor - Hamer (1750 rpm/110volts). The employer had not established a written energy control procedures for machines and equipment, employee training, and periodic inspections. Employees were thereby exposed to the hazards of moving and rotating equipment.

Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.    a. Bagging Building/Floor surface next to metal drain - On or about October 28, 2011, there was a hole (12' by 5 1/2" by 1 1/2") in the working floor of the production area, thereby exposing employees to slip, trip, and fall hazards.

Outside yard and inside the Bagging Building - On or about October 28, 2011, employees producing landscaping materials were required to operate diesel powered industrial trucks that were unsafe and not withdrawn from service. The diesel powered industrial trucks were the following;    1. The Toyota fork lift truck's (# 17 Serial # 60540 and Model # 7FDU25) fastening system was missing its flat cable component.    2. The Toyota fork lift truck's (# 18 Serial # 60452 and Model # 67DU20) fastening system was missing its flat cable component.

Entire Facility - On or about October 28, 2011, the employer's OSHA 300 Log forms were missing information on column F.  The entries per year that were missing information were the following;    1. Three case entries in the OSHA 300 Log form for the year 2009 were missing information such as the object/substance that directly injured or made person ill.    2. Two case entries in the OSHA 300 Log form for the year 2010 were missing information such as the object/substance that directly injured or made person ill.    3.  One case entry in the OSHA 300 Log form for the year 2010 was missing information such as the description of the injury or illness and the parts of body affected.    4. One case entry in the OSHA 300 Log form for the year 2010 was missing information such as the description of the injury or illness.
