29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level: a.) On or about September 22, 2011, at the jobsite located at 23180 Volbrecht Road, Crete, IL - employees were performing dryvit work from a tubular welded frame scaffold that was not equipped with guardrails, thereby exposing the employees to approximately ten (10) feet, eleven (11) inches fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. a.) On or about September 22, 2011, at the jobsite located at 23180 Volbrecht Road, Crete, IL - employees performing dryvit work from a tubular welded frame scaffold was not provided with an additional ladder to safely access the scaffold, thereby exposing the employees to approximately ten (10) feet, eleven (11) inches fall hazards.
