29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. On or about December 12, 2012, at the above addressed jobsite, employees were exposed to falls while performing dryvit operations from a mobile tubular welded frame scaffold, eleven (11) feet four (4) inches above the ground, that did not have the working level fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. a. On or about December 12, 2012, at the above addressed jobsite, employees conducting dryvit operations from a mobile tubular welded framed scaffold, eleven (11') feet four (4") inches above the ground, were not provided with an access ladder, thereby exposing the employees to falls. THE PAUL BUILDING IMPROVEMENT, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(e)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 108483, CITATION NUMBER 1, ITEM NUMBER 1a AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 14, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 23180 VOLBRECHT ROAD, CRETE, IL 60417.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part: a. On or about December 12, 2012, at the above addressed jobsite, employees performing dryvit operations from a mobile tubular welded frame scaffold, eleven (11) feet four (4) inches above the ground, were exposed to injuries from falls due to the lack of guardrails.
