29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: (a) At the project site roof, a mason was applying wet Hansen Portland cement between 2 X 4 inch forms without the use of protective gloves. Employee was exposed to the hazards of wet Hansen Portland cement contacting the skin thereby, potentially damaging the skin because it is caustic, abrasive, and absorbs moisture. JIAN HUA CONSTRUCTION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.95(a) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315092478, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON FEBRUARY 3, 2011.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) At the project site roof, a mason was applying wet Hansen Portland cement onto the roof without the use of eye protection. The mason was exposed to the hazards of wet Hansen Portland cement contacting his eyes. JIAN HUA CONSTRUCTION, INC. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) At the project site roof, an extension cord without strain relief was used to connect a Hilti SD 4500 screw gun. The screw gun was used to secure 2 X 4 inch forms onto the roof. JIAN HUA CONSTRUCTION, INC.
