(a) At Barracks 9, 1st floor, eye protection was not used when applying wet "Polyblend" sanded grout. Employee was exposed to the hazard of wet or dry grout contacting the eyes.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace: (Construction Reference: 1926.59) At the work site, on October 21, 2011, and at times prior thereto, material safety data sheets were not readily available for hazardous materials such as, but not limited to Polyblend sanded grout.
