29 CFR 1910.303(b)(2): 'Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. (a) Electronics counter: On or about November 3, 2011 and times prior thereto, relocatable power taps (rpts) were plugged into one another. This exposed employees to fire/smoke inhalation hazards from over loading the RPTs and starting a fire. This exposed employees to fire/smoke inhalation hazards from over loading the RPTs and starting a fire. 29 CFR 1910.303(g)(1): 'Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. Warehouse: On or about Novemebr 3, 2011 and times prior thereto, the breaker panel where bikes were assembled was blocked. This exposed employees to tripping hazards and delays in accessing the panel in the event of an emergency.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. Warehouse: On or about November 3, 2011 and times prior thereto, the relocatable power tap was hanging by its cord exposing the wires. This exposed employees to shock hazards from accidental contact.
