29 CFR 1910.151(b):     In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is usedfor the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.      Warehouse:  On or about November 2, 2011 and at times prior thereto, where employees utilized boxcutters, persons trained in first aid were not available.  This condition prevented employees from receiving prompt medical attention from significant laceration hazards.

29 CFR 1910.176(a):     Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.      Warehouse:  On or about November 2, 2011 and at times prior thereto, items including ladder stands, merchandise carts, and boxed merchandise were allowed to accumulate in aisles and passageways, reducing width(s) to 18 inches or less.  This condition exposed employees to smoke inhalation hazards from delay in exiting the facility.

29 CFR 1904.29(a):     Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    Coast Guard Exchange:  On or about November 2, 2011 and at times prior thereto, where there were recordable work-related injuries and illnesses for 2011, the employer did not maintain an OSHA 300 or equivalent form.
