Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes:    Building N1, Vehicle Maintenance:  On or about October 31, 2011 and at times prior thereto, employee utilized polyvinyl chloride(PVC)-coated and cracked rubber-coated gloves when handling paint reducers and thinners.  These gloves were not appropriate for the specific tasks and would not provide a protective barrier.  This condition exposed employees to skin contact and skin absorption hazards to the chemical constituents of the paint products.

These include but are not limited to PP-2, water pump; PP-9, treated oil pump; and, PP-11, liquid oily waste pump.  This condition exposed employees to struck-by, caught-in, and electrical shock and burn hazards in the event of unexpected start-up during servicing and maintenance.    (b)  Building N21, Fuel Operations:  On or about November 1, 2011 and at times prior thereto, specific procedures for controlling potentially hazardous energy had not been developed for specific pumps and filter units.  These include but are not limited to hydrant system pumps, fill stand pumps, and filter pressure vessels.  This condition exposed employees to struck-by, caught-in, and electrical shock and burn hazards in the event of unexpected start-up during servicing and maintenance.
