29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Pipe Shop; on or about 21 October 2014 and times prior thereto; the work rest on a Stanley bench grinder type 610, Serial number G28400E was not adjusted closely to the abrasive wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the abrasive wheel and the work rest. Work rest was approximately 1/4 of an inch from the abrasive wheel.
