29 CFR 1910.132(c):     Design. All personal protective equipment shall be of safe design and construction for the work to be performed.    Steam Plant:  On or about November 1, 2011 and times prior thereto, employees were using a 1/2 screw pin shackle as part of their fall arrest system.  This exposed employees to fall hazards from using a shackle that was not designed or tested as part of a fall arrest system.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc    Building 19:  On or about November 1, 2011 the Oster Williams pipe threading machine had no guards installed.  This exposed employees to caught-in/struckby hazards from being at the point of operation or walking by the machine.

29 CFR 1910.213(a)(12):     For all circular saws where conditions are such that there is a possibility of contact with the portion of the saw either beneath or behind the table, that portion of the saw shall be covered with an exhaust hood, or, if no exhaust system is required, with a guard that shall be so arranged as to prevent accidental contact with the saw.    Shipping and Receiving:  On or about October 31, 2011 and times prior thereto the Delta table saw S/N 87E69415 had an inoperative guard.  This exposed employees to amputation hazards to the hands from accidentall contact with the blade during cutting operations.

(a)  Warehouse Area 1st floor:  On or about 10/31/2011, and times prior thereto, a portable fire extinguisher was mounted in the work area, not marked or identified as being out of service in a discharged condition.    (b)  Building N-1:  On or about 10/31/2011, and times prior thereto, a portable fire extinguisher was laying in the shop area on the ground, not marked or identified as being out of service in a discharged condition.    (c)  Building N-1A:  On or about 10/31/2011, and times prior thereto, 3 portable fire extinguishers were laying in the welding shop area on the ground, not marked or identified as being out of service; 1 of which was in a discharged condition.      (d) Building 19:  On or about 10/31/2011, and times prior thereto, a portable fire extinguisher was not mounted in the work area.

29 CFR 1910.179(h)(4):     Hooks. Hooks shall meet the manufacturer's recommendations and shall not be overloaded.    Building 19:  On or about November 1, 2011 and times prior thereto, the DZ chain hoist did not have a latch installed on the upper hook.  This exposed employees to possible struck-by hazards if the load were to disengage from the hook since there was no latch to stop it.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    Auto repair shop:  On or about October 31, 2011 and times prior thereto, employees were using an air nozzle that was operating at 110 psi.  This exposed employees to laceration or air embolism hazards if it were to contact their body.
