29 CFR 1910.146(g)(1):  The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146, acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned:    a)  Cloverleaf Cold Storage Company  On or about November 10, 2011, the employer did not assure each employee that was required to enter a permit required confined space was trained with the knowledge and skills necessary to safely perform entry of a permit required confined space of the meat massage unit MA 12000SD.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) Cloverleaf Cold Storage Company - On or about November 10, 2011, the employer did not assure that employees who perform cleaning operations on the meat massage unit MA 12000SD were trained in the skills required for the safe application, usage, and removal of the energy controls.  Employees that clean the massage unit were not trained to lockout the machine to protect against struck-by hazards.

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by authorized employees performing servicing or maintenance:      a) Cloverleaf Cold Storage Company - On or about November 10, 2011, the employer did not assure that each authorized employee placed a lockout device on the energy disconnect on the meat massage unit MA 12000SD prior to cleaning.  A supervisor applied their locks and tags on the electrical disconnect while another employee entered into the unit to perform cleaning.

29 CFR 1910.146(c)(7)(iii):  The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination:    a) Cloverleaf Cold Storage Company - On or about November 10, 2011, the employer did not document and certify the reason in which the space associated with the meat massage unit MA 12000SD was reclassified to a non-permit required confined space.  The multiple energies associated with the unit have to be locked out prior to entering.
