29 CFR 1910.178(m)(10): Load backrest extension(s) were not used on industrial trucks to minimize the hazard of rearward falling materials:   A)    Cloverleaf Cold Storage Company, located at 1165 Independence Drive, Napoleon, OH 43545: On or about July 31, 2014, the employer did not ensure that the Crown forklift (Model SC4520-35, Serial # 9A156274) was equipped with a load backrest extension in order to minimize the possibility of the load or part of the load from falling rearward. The material being lifted by the forklift operators exceeded the height of the forklifts load backrest by approximately 4 feet, exposing employees in the area to struck-by hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A)    Cloverleaf Cold Storage Company, located at 1165 Independence Drive, Napoleon, OH 43545: On or about July 30, 2014, the employer failed to ensure that the Clarke Metalworker 16-Speed Drill Press (Model BT1029), located in the facilitys maintenance area, was properly guarded in order to prevent employees from inadvertently being caught by the rotating chuck during the drill press operation.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  A)    Cloverleaf Cold Storage Company, located at 1165 Independence Drive, Napoleon, OH 43545: On or about July 30, 2014, the employer did not ensure that the left side of the Dayton 6 Bench Grinder (Model 4Z123G, Lot # 200110), located in the facilitys maintenance area, was equipped with a work rest that was adjusted to 1/8 inch away from the abrasive wheel. Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheel breakage.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.215(d)(1): Wheel(s) on grinding machine(s) were not sounded (ring test) and inspected for damage before mounting:   A)    Cloverleaf Cold Storage Company, located at 1165 Independence Drive, Napoleon, OH 43545: On or about July 30, 2014, the employer did not ensure that employees closely inspected and sounded (ring test) the abrasive wheel immediately before mounting it on the Dayton 6 Bench Grinder (Model 4Z123G, Lot # 200110) in order to make sure that the wheel has not been damaged in transit, storage, or otherwise. Employees that operated the grinder were exposed to injuries from potentially defective abrasive wheel and/or to struck-by hazards in a case of the wheel breakage.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheel breakage.
