29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      (a)  Employees are exposed to crushing hazards while servicing and maintaining equipment and the employer did not develope a written energy control (Lockout/Tagout) program.

29 CFR 1910.217(b)(7)(x):     Foot operated control(s) used on mechanical power press(es) with part revolution clutches were not protected to prevent operation from falling or moving objects, or from unintended operation by accidental stepping onto the foot control.      (a)  Employees were exposed to amputation hazards while the Biss 39 BM press, which was located in the machine shop, and the employer did not ensure that foot pedal was protected from unintentional operations.

(b)  Employees were exposed to amputation hazards while operating the Bliss (model Number 39 BM) part revaloution power press and the employer did not ensure that that the point of operation was properly guarded.
