29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 22, 2011, the employer did not assure that employees were protected from falls when climbing the load side, adjacent to the control panel, of Line 4 to retrieve bars before they are moved into the oven.  Employees that climbed the frame were exposed to a fall hazard of approximately 68-inches.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.132(d)(1)(i): The employer did not select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment:    a) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 22, 2011, the employer did not perform an adequate hazard assessment to determine the proper protective equipment to be worn by employees exposed to hazards associated with Line 4.  Employee(s) operating that line were exposed to falls when retrieving bars before they entered the oven.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were exposed to crushing injuries in that energy control was not utilized before employees entered hazard areas to change parts known as seats.      b) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 30, 2011, the employer did not assure that machine specific energy control procedures were developed, documented and utilized to control hazardous energy when employees entered the compression section of Line 4 to perform set up activities.  Employees were exposed to crushing injuries in that energy control was not utilized before employees entered hazard areas to change parts known as seats.      c) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 22, 2011, the employer did not assure that machine specific energy control procedures were developed, documented and utilized to control hazardous energy when employees accessed the Line 1 coiler area to perform set-up and changeover activities.  Employees were exposed to  pinch point, rotating parts, and amputation injury risk in that energy control was not utilized before employees accessed the hazard areas.

Employees that service Line 1 were not trained to lockout the machine to protect against crushing hazards.      b) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 30, 2011, the employer did not assure that employees who perform set-up and changeover activities on the rams in the compression section of Line 4 were trained in the skills required for the safe application, usage, and removal of energy controls. Prior to accessing the hazard areas to perform the changeover activities, the employer failed to implement energy control application steps as required by paragraph (d) of this section - the employer did not:        -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).       -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.       -  Verify that isolation and deenergiziation of the machine or equipment had been accomplished.    b) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 30, 2011, employee(s) were exposed to a crushing hazard in that the break down ram located at the cold set section of Line 4 was not locked out prior to changing ram seats.  Prior to accessing the hazard areas to perform the changeover activities, the employer failed to implement energy control application steps as required by paragraph (d) of this section - the employer did not:        -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).       -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.       -  Verify that isolation and deenergiziation of the machine or equipment had been accomplished.    c) Rassini Chassis Systems, LLC - Montpelier, OH: On or about November 22, 2011, employee(s) were exposed to pinch point, rotating parts, and amputation injury risk in that the coiler of Line 1 was not locked out prior to performing set-up and changeover activities.
