Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible metal dust deflagration and explosion hazards while working at or near filter media dust collectors and associated equipment which were not adequately designed to prevent or minimize employee exposure in the event of a deflagration:    (a)	Rassini Chassis Systems, LLC, 1812 Magda Drive, Montpelier, Ohio:  On or about January 31, 2012, the employer did not ensure that employees were protected from dust deflagration and explosion hazards.  The employees were exposed to hazards from a filter media dust collector, located inside the building and connected to the #1 abrasive blast line, which lacked adequate means of (1) explosion protection, and (2) deflagration isolation protection to upstream process equipment.    (b)	Rassini Chassis Systems, LLC, 1812 Magda Drive, Montpelier, Ohio:  On or about January 31, 2012, the employer did not ensure that employees were protected from dust deflagration and explosion hazards.  The employees were exposed to hazards from a filter media dust collector, located inside the building and connected to the #3 abrasive blast line, which lacked adequate means of (1) explosion protection, and (2) deflagration isolation protection to upstream process equipment.    Among other methods, a feasible method to correct this hazard would be to comply with National Fire Protection Association (NFPA) Standards 484 "Standard for Combustible Metals, 2006 Ed." - Sections 12.2.1.4, 12.2.10.1, 12.2.10.7, 12.2.10.8, 13.3, NFPA 654 "Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids, 2006 Ed." - Sections 7.1.2, 7.1.4, 7.1.5, 7.13.1.1.1, 7.13.1.3.1,  7.13.1.5, and/or to comply with FM Global Property Loss Prevention Data Sheet 7-73 "Dust Collectors and Collection Systems, May 2010" - Section 2.1.1.1, 2.1.2.2, 2.1.4.2 and 7-76 "Prevention and Mitigation of Combustible Dust Explosions and Fire, May 2009" - Sections 2.4.1.1

29 CFR 1910.1200(h)(1):  The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:      (a)	Rassini Chassis Systems, LLC, 1812 Magda Drive, Montpelier, Ohio:  On or about January 31, 2012, the employer did not ensure that employees were provided with information in the safe handling of combustible dusts.  Employees were not aware of the fire and explosion hazards associated with combustible dusts.
