29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Rassini Chassis Systems, LLC - Montpelier, Ohio: On or about March 11, 2016, employees were exposed to amputation, pinch point, and struck-by hazards in that the Line 4 Draw Robot Loader (s/n 8092915) was not locked out prior to servicing the robot's end effector.  Energy control application steps to control the electrical and pneumatic energy sources were not implemented prior to accessing hazard areas of the machine for servicing activities; the employer did not:   - Physically operate energy isolating devices to control energy sources and affix lockout device(s).  - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.   - Verify that isolation and deenergiziation of the machine or equipment had been accomplished.    b. Rassini Chassis Systems, LLC - Montpelier, Ohio: On or about March 15, 2016, each authorized employee did not affix lockout devices to the electrical and pneumatic energy isolating devices for the Line 4 Draw Robot Loader (s/n 8092915) before employees serviced the robot's end effector.  A potential for unexpected startup of the machine existed while an authorized employee serviced the machine.    Rassini Chassis Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 109320, citation number 1, item number 4 and was affirmed as a final order on February 14, 2012, with respect to a workplace located at 1812 Magda Dr. in Montpelier, Ohio, 43543.
