29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  On 11-22-11, employees were working on a steep slope main house roof with an eave edge greater than twelve (12) feet above compacted surface and a trash hopper without any fall protection equipment.    b)  On 11-222-11, employees were working on a low slope porch roof with an eave edge greater than twelve (12) feet above compacted surface without any fall protection equipment.      No Abatement Certification or Abatement Documentation required

29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    Employer did not provide certification of employee fall protection training for employees who did not recognized fall from elevation hazard associated with working on a roof at a height greater than six feet without utilizing fall protection equipment. 29 CFR 1926.1053(b)(1):     Portable ladder was used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees who access/egress a roof work surface at a height greater than twelve (12) feet were utilizing a Werner 24 foot extension ladder without extending the side rails at least three (3) feet above the upper landing surface.      No Abatement Certification or Abatement Documentation required
