29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     a) 325 W. Glenwood Avenue, Smyrna, DE - Employees were performing roofing activities on a commercial construction site, with unprotected sides and edges, approximately 20 feet above the lower level, without any means of fall protection.  The employees were installing sheathing on the 8/12 pitch roof.  The condition was observed on March 9, 2015.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:      a) The employer did not provide a training program for employees to recognize and minimize fall hazards in construction activities.  The employees were working on the roof, approximately 20 feet above the lower level, without any means of fall protection.  The employees were not aware of procedures to minimize fall hazards, and alternative means of fall protection or equipment.  The condition was documented on March 9, 2015.     Abatement Note    The training program must enable the employees to recognize the hazards of falling and must train each employee in the procedures to be followed in order to minimize these hazards.  Each employee must be trained by a competent person in the following:     1. The nature of fall hazards in the work area;    2. The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;  3. The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety         monitoring systems, controlled access zones, and other protection to be used;  4. The role of each employee in the safety monitoring system when this system is used;  5. The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs;  6. The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection;    7. The role of employees in fall protection plans;  8. The standards contained in this subpart.     The employer must verify compliance with paragraph (a) of this section by preparing a written certification record that will contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:      a) 325 W. Glenwood Avenue, Smyrna, DE - Employees were observed using Werner aluminum extension ladders that were not extended at least 3 feet above the upper landing to access the roof.  The employees used the ladders for roof access to install sheathing.  This exposed the employees to fall hazards over 10 feet above the grade.  The condition was observed on March 9, 2015.

Glenwood Avenue, Smyrna, DE - The employer did not provided ladder hazard recognition training for each employee.  The employees used ladders unsafely to access the roof and install sheathing.  The condition was documented on March 9, 2015.
