29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: At Shell Yigo, the double-locking snaphook nearest to the deceleration device of the "SALA" lanyard, serial number 2443879, was not inspected prior to each use for wear, damage and other deterioration. The inner spring of the double-locking snaphook was defective, thereby potentially resulting in roll out and disengagement from the Dorsal D-Ring.
