29 CFR 1926.1053(b)(7):     Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:    a) Roof of American Bakery Building: On or about 12 DEC 11 and at times prior thereto, workers were using a stepladder, with the spreaders in the closed position to ascend and descend from their work area. The stepladder was used in the folding position and was not tie off exposed workers to a fall hazard.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:    a) Roof of American Bakery Building: On or about 12 DEC 11 and at times prior thereto, workers were using a stepladder, with the spreaders in the closed position to ascend and descend from their work area. The workers lack the training as the stepladder was being used in the folding position exposed workers to a fall hazard.
