29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2 1969: On or about 12/12/11 during construction activities, the employer did not ensure it had the manufacturers' approval to modify an aerial lift for material lifting and positioning of metal panels.
