29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) At the facility, the #12 Bergandi wire chain link machine's wire shear was not guarded exposing employees to an amputation hazard. b) At the facility, the #12 Bergandi wire chain link machine's forming tool that was forming a bar band on the end of the chain link fencing was not guarded exposing employees to an amputation hazard. c) At the facility, the #12 Bergandi wire chain link machine's forming tool that was forming a knuckle band on the end of the chain link fencing was not guarded exposing employees to an amputation hazard.
