Lake Street, Melrose Park, IL - On or about November 14, 2011, during removal of asbestos-containing transite panels from the diesel shop, M & O Environmental Company did not provide and display signs in accordance with 29 CFR 1926.1101(k)(7).    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Lake Street, Melrose Park, IL - On or about November 14, 2011, during removal of asbestos-containing transite panels from the diesel shop, M & O Environmental Company did not obtain breathing zone air samples that were representative of the 8-hour time-weighted average and 30-minute short-term exposures to determine the exposures of employees who removed the transite panels.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.1101(g)(3)(i):     High-speed abrasive disc saws were not equipped with point of cut ventilator or enclosures with HEPA filtered exhaust air:    a) 5050 W. Lake Street, Melrose Park, IL - On or about November 14, 2011, during removal of asbestos-containing transite panels from the diesel shop, M & O Environmental Company employees used a Metabo WEP 14-150 QuckProtect angle grinder with a Metabo Slicer Plus Reinforced Type 1 Cutting Wheel without a point of cut ventilator or an enclosure with HEPA filtered exhaust air when cutting bolt heads that secured the transite panels.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1101(j)(2)(i):     The employer did not establish a decontamination room or area for employees and equipment, adjacent to the regulated area, consisting of an impermeable drop cloth on the floor or horizontal working surface, for employees engaged in Class I work involving less than 25 linear or 10 square feet of thermal system insulation or surfacing asbestos containing material and for Class II and Class III asbestos work operations where exposures exceed a permissible exposure limit or where there was no negative exposure assessment produced before the operation:    a) 5050 W. Lake Street, Melrose Park, IL - On or about November 14, 2011, in the absence of a negative exposure assessment, M & O Environmental Company, when removing asbestos-containing transite panels from the diesel shop, did not establish a decontamination room or area that contained an impermeable drop cloth on the floor.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
