29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) Radical Roofing, Oregon, Ohio:  On or about December 22, 2011, at address 3728 Starr Ave., Oregon, Ohio, the employer failed to ensure that each employee was protected from head injuries while working below employees conducting roofing work above.  Employees were exposed to potential impact injuries caused from falling materials and/or equipment while picking up scrap materials from the ground below the working level.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above the lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems nor was another provision in paragraph (b) of this section provided:    a) Radical Roofing, Oregon, Ohio:  On or about December 22, at address 3728 Starr Ave., Oregon, Ohio, the employer did not ensure that employees were protected from falls while removing old roofing material from a steep sloped roof (8:12) on a residential type structure.  Employees were exposed to a potential fall of approximately 17 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) Radical Roofing, Oregon, Ohio:  On or about December 22, 2011, at address 3728 Starr Ave., Oregon, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems.  Employees were exposed to falls of approximately 17 feet to the ground below, which could result in permanent disability or death.
