29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Radical Roofing, Oregon, Ohio: On or about December 22, 2011, at address 3728 Starr Ave., Oregon, Ohio, the employer failed to ensure that each employee was protected from head injuries while working below employees conducting roofing work above. Employees were exposed to potential impact injuries caused from falling materials and/or equipment while picking up scrap materials from the ground below the working level.

Employees were exposed to a potential fall of approximately 17 feet to the ground below.
