29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: A. Radical Roofing worksite located at 4139 Stonehenge, Sylvania, Ohio: On or about May 14, 2013, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the two story multi-family residential structure. Employees were exposed to a fall distance of approximately 20.00 feet from the 4/12 pitched roof. Radical Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501, which was contained in OSHA inspection number 110616, citation number 1, item number 2 and was affirmed as a final order on January 19, 2012, with respect to a workplace located at Oregon, Ohio.
