29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: (a) Two employees that were servicing Machine #125 F & E Printer, were using the same lock for performing lockout/tagout. Only one employee had control of the key for the lock. The employee with the key could start up the machine without the other employee knowing exposing that individual to caught-in-between, crushing, or amputation hazards.

(b) Machine #125 F & E Printer, Model PB, Serial #1062, had multiple rollers that were not guarded exposing employees to caught-in-between and possible amputation hazards. (c) Machine #169 Interlake Stitcher, Model 05POS, Serial #1055 had moving parts that were not guarded exposing employees to caught-in-between hazards.
