(a) In the production area, the employer did not ensure the Bobst #145 flat die cutter had in-running nip points on the discharge conveyor effectively guarded. Employees were exposed to entanglement hazards. (b) In the production area, the employer did not ensure that guard on the Primo 36 RT cardboard-cutting band saw was adjusted to cover the unused portion of the blade.
