29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) On or about 12/14/2012, the Cornell trim saw was not turned off or shut down using the procedures established for the machine when an employee reached into the conveyor area to unjam a piece of wood which exposed the employee to a moving saw blade which resulted in broken and/or amputated fingers. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
