29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. A portable Lincloln Arc Welder Model SAE-400 was in use and several extension cords were connected powering tools on the construction site. The Welder was not equiped with a ground fault circuit interrupter.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: A damaged extension cord was in use which had a severe cut due to rubbing against a sharp object. Cord in use was not being used with a ground fault circuit interrupter (GFCI).
